Notifiable Diseases

Disease

Case Identification Logic

Chlamydia

Positive result on any of the following tests:
-Chlamydia trachomatis culture
-C. trachomatis nucleic acid probe, nucleic acid amplificiation assay, or enzyme immunoassay

 

Gonorrhea

 

Positive result on any of the following tests:
-Neisseria gonorrhoeae culture
-N. gonorrhoeae nucleic acid probe, nucleic acid amplification assay, or enzyme immunoassay

 

Pelvic inflammatory disease

 

Diagnosis with any of the following ICD-9 codes:
-614.0 – acute salpingitis or oophoritis
-614.2 – salpingitis or oophoritis, not otherwise specified
-614.3 – acute parametritis and pelvic cellulitis
-614.5 – acute pelvic peritonitis
-614.9 – pelvic inflammatory disease
-099.56 – C. trachomatis infection of the peritoneum

And at least one of the following within 28 days before or after diagnosis with the ICD-9 code:
-Meets the ESP case definition for C. trachomatis
-Meets the ESP case definition for N. gonorrhoeae

 

Acute hepatitis A

 

Either of the following:
-Alanine aminotransferase or aspirate aminotransferase greater than two times the upper limit of normal
- ICD-9 code 782.4 for jaundice

 

And the following within a 14-day period:
-Positive immunoglobulin M (IgM) antibody to hepatitis A virus

 

Acute hepatitis B

 

Both of the following:
-ICD-9 code for jaundice OR liver function tests > 5x normal
-IgM to core antigen

OR

All four of the following:
-ICD-9 code for jaundice OR liver function tests > 5x normal
-Hep B surface antigen or ‘e’ antigen present
-No prior positive Hep B specific lab tests
-No present or prior ICD-I code for chronic hepatitis B

 

Active Tuberculosis

 

Any one of the following:
-Prescription for pyrazinamide
-Order for AFB smear or AFB culture followed by ICD-9 code for TB within 60 days
-Order for 2 or more anti-TB medications followed by an ICD-9 code for TB within 60 days

Frank Diabetes

Any of the following:
-Hemoglobin A1c  > 6.5%
-Fasting glucose > 126 mg/dL
-Prescription for insulin outside pregnancy
-ICD-9 code 250.xx on two or more occasions
-Prescription for one or more of the following medications
glyburide, gliclazide, glipizide, glimepiride
pioglitazone, rosiglitazone
repaglinide, nateglinide, meglitinide
sitagliptin
exenatide, pramlintide

 

Type I

 

Meet frank diabetes criteria with any of the following:
-C-peptide negative
-DM auto-antibodies positive
-Prescription for urine acetone test strips
-Ratio of type 1 : type 2 diabetes ICD9s > 0.5 and NOT on oral hypoglycemic
-Ratio of type 1 : type 2 diabetes ICD9s > 0.5 and prescription for glucagon

 

Type II

 

Meet frank diabetes criteria and do not meet any type I criteria
